Guy's and St Thomas' NHS Foundation Trust intends to award a contract to Age UK Lambeth for the provision of a Social Prescribing Link Worker embedded within the Lambeth Ageing Well Integrated Neighbourhood Team. The service supports older residents living with, or at risk of, frailty. It provides personalised non clinical support, advice and signposting through home visits and community outreach, connecting residents with local voluntary and community services. The link worker supports multidisciplinary working, referral follow up and the development of the neighbourhood prevention model.

The service will provide an embedded social prescribing link worker to identify residents' social, practical and wider wellbeing needs, provide personalised advice, signposting and support through home visits and community outreach, connect residents with relevant services, work collaboratively with primary care and other partners.
