Foundations for Change Flexible Framework for Additional Support Services (Adults Aged 18–65)
Lot 1 – Learning Disabilities (including complex needs and behaviours that challenge)
Provision of additional support services for adults aged 18–65 with learning disabilities, including those with complex needs and behaviours that challenge. Services will be person centred, outcomes focused and delivered in line with Self Directed Support Option 2 and Option 3. Providers will support individuals to live independently within their communities and work in partnership with the HSCP and other agencies. Services must be capable of delivery on a 24 hours a day, 7 days a week, 365 days a year basis where required. The duration of the framework exceeds four years due to the complex and long term nature of the services being delivered. Continuity of care for vulnerable individuals, workforce stability and the need for providers to invest in sustainable service delivery models require a longer term arrangement. The flexible structure of the framework ensures ongoing competition and responsiveness to changing needs.
Lot 2 – Physical Disabilities
Provision of additional support services for adults aged 18–65 with physical disabilities. Services will support individuals to maximise independence and achieve personal outcomes, including delivery of personal care, mobility support and community engagement. Services will be delivered in line with Self Directed Support Option 2 and Option 3 and must reflect a person centred, outcomes based approach. Services must be capable of delivery on a 24 hours a day, 7 days a week, 365 days a year basis where required. The duration of the framework exceeds four years due to the complex and long term nature of the services being delivered. Continuity of care for vulnerable individuals, workforce stability and the need for providers to invest in sustainable service delivery models require a longer term arrangement. Lot 3 – Cognitive Impairment (including Acquired Brain Injury)
Provision of additional support services for adults aged 18–65 with cognitive impairments, including acquired brain injury. Providers will deliver tailored support to meet complex cognitive and behavioural needs, supporting individuals to maintain independence and achieve agreed outcomes. Services will be delivered in line with Self Directed Support Option 2 and Option 3 and must be flexible and responsive, including provision on a 24 hours a day, 7 days a week, 365 days a year basis where required. The duration of the framework exceeds four years due to the complex and long term nature of the services being delivered. Continuity of care for vulnerable individuals, workforce stability and the need for providers to invest in sustainable service delivery models require a longer term arrangement. Lot 4 – Sensory Impairment
Provision of additional support services for adults aged 18–65 with sensory impairments. Services will support individuals with visual and or hearing impairments to live independently and access their communities. Support must be tailored to individual needs and delivered in line with Self Directed Support Option 2 and Option 3 using a person centred, outcomes based approach. Services must be available on a 24 hours a day, 7 days a week, 365 days a year basis where required. The duration of the framework exceeds four years due to the complex and long term nature of the services being delivered. Continuity of care for vulnerable individuals, workforce stability and the need for providers to invest in sustainable service delivery models require a longer term arrangement. Lot 5 – Mental Health (including severe and enduring conditions)
Provision of additional support services for adults aged 18–65 with mental health conditions, including severe and enduring needs. Services will promote recovery, independence and social inclusion, supporting individuals to achieve outcomes aligned to their care plans. Services will be delivered in line with Self Directed Support Option 2 and Option 3 and must be responsive and available on a 24 hours a day, 7 days a week, 365 days a year basis where required. The duration of the framework exceeds four years due to the complex and long term nature of the services being delivered. Continuity of care for vulnerable individuals, workforce stability and the need for providers to invest in sustainable service delivery models require a longer term arrangement. The flexible structure of the framework ensures ongoing competition and responsiveness to changing needs.
Lot 6 – All Services (Lots 1–5)
Provision of additional support services across all service areas outlined in Lots 1 to 5. Providers must demonstrate the capability to deliver services across multiple client groups including learning disabilities, physical disabilities, cognitive impairments, sensory impairments and mental health. Services must be delivered in line with Self Directed Support Option 2 and Option 3 using a person centred, outcomes focused approach. Services must be flexible and available on a 24 hours a day, 7 days a week, 365 days a year basis where required. The duration of the framework exceeds four years due to the complex and long term nature of the services being delivered.
