Royal Free London NHS Foundation Trust is conducting a procurement for an Enhanced Virtual Care programme using the Competitive Process under the Health Care Services (Provider Selection Regime) Regulations 2023.

The Service provides digitally enabled proactive care for people with complex long-term conditions who are at increased risk of deterioration and avoidable admission. A third party is sought to help deliver remote monitoring, structured clinical triage, personalised care planning, health coaching and medicines optimisation, with agreed escalation routes into primary, community, virtual ward and acute services.

This is a Provider Selection Regime (PSR) Contract Notice. The awarding of this contract is subject to the Health Care Services (Provider Selection Regime) Regulations 2023. For the avoidance of doubt, the provisions of the Procurement Act 2023 do not apply to this award.

Royal Free London NHS Foundation Trust is conducting a procurement for an Enhanced Virtual Care programme using the Competitive Process under the Health Care Services (Provider Selection Regime) Regulations 2023.

This procurement follows an earlier market engagement exercise undertaken by the Trust to understand provider capability and inform the development of the service specification and commercial model. This engagement did not form part of a provider selection process and no provider was selected as a result of it.

The Service provides digitally enabled proactive care for people with complex long-term conditions who are at increased risk of deterioration and avoidable admission. A third party is sought to help deliver remote monitoring, structured clinical triage, personalised care planning, health coaching and medicines optimisation, with agreed escalation routes into primary, community, virtual ward and acute services.

A detailed analysis was undertaken of population segmentation including patient level costs across acute, community, mental health and primary care settings. Patients were also segmented based on the highest cost conditions (i.e cardio, respiratory) This led to the best potential indicators of future patient cohorts being in the top 5% spend in the next 12 months.

Patient led remote onboarding of patients and baselining of key physiological parameters, development of a jointly held care plan, structured action to address identified care gaps such as medicines optimisation and vaccination, access to lifestyle-based interventions and direct access to appropriate clinical support and onward escalation were required.
