Intention notice Jet ventilator for St. Olav's hosptial

St. Olavs Hospital has decided to procure one Jet ventilator; The estimated value of the procurement, including equipment dependent consumables in the equipment ́s lifetime, is NOK 1,000,000,000. NOK 1,200,000 excluding VAT. Justification procurement without competition: This intention notice is anchored in the procurement regulations § 13-4 letter b no. 2 on situations where competition is impossible for technical reasons. The ENT department currently has a Monsoon 3 jet ventilator that is approx. 10 years old. This has, in recent years, had repeated technical problems, and consumables have been difficult to procure. It has not been possible to treat patients with narrow stenoses or high BMI with the current jet ventilator, which has led to increased costs by sending these patients to Rikshospitalet-OUS for treatment. Rikshospitalet has now refused to receive several patients due to capacity challenges and informed that this patient group must be treated at the university hospitals. As a university hospital, St. Olavs Hospital is responsible for advanced airway surgery in the Mid-Norway Health Trust. When patients can no longer be transferred to Rikshospitalet, the hospital must therefore have equipment that allows treatment at equivalent professional levels locally. The Twin Stream jet ventilator is decisive for treating patients with high BMI, reduced lungcompliance and very tight subglottic stenosis and to avoid the risk of peroperative hypoxi and hypercapnia. It has high frequency and the possibility for superimposed high frequency jet ventilation which provides more stable ventilation and oxygenation. This is decisive for patients with excess weight and/or narrow stenosis in the respiratory tract, which is the most critical patient group. With the highest frequency that TS uses, in addition the movements in the operating field will be minimised in relation to jet ventilation with a lower frequency. The surgeon will have better working conditions, which reduces operational time and makes complex interventions easier and safer. The integrated jet system in the laryngoscopes also provides a good overview of operational fields and instrument access. Only TS enables oxygenation/ventilation of overweight patients and particularly tight stenoses, which have a high enough frequency to achieve a stable surgical field and have integrated jet nozzles in the laryngoscopes. The Twin Stream is used at the largest Norwegian and foreign centres, including the National Hospital in Oslo. Access to the same technology provides a joint competence platform, simple cooperation between the centres and easier training of surgeons and anaesthesia personnel. It is crucial for us in the treatment of such complex patients to have the same type of equipment, when we shall consult and discuss with, for example, Rikshospitalet.Based on a market analysis, only Simonsen & Weel AS can deliver the solution in question based on St.Olav Hospital ́s needs and our assessment is that the terms to hold a competition without a prior notice in accordance with the procurement regulations § 13-4 letter b no. 2 are met.
