A Kirklees Adult Carers Information, Advice and Advocacy Service to work co-productively with Adult Carers to promote, support, and improve their mental, physical and emotional well-being by identifying and achieving personal outcomes that enable Adult Carers to continue in their caring role positively and safely, reducing the need for more costly health or social care interventions.
