It is nationally recognised that service users with serious mental illness or learning disability are more likely to have co-morbid physical health issues and shorter life expectancy. Residing in inpatient services for long periods of time can also result in service users being more likely to develop long term physical health conditions. A number of service users within each of the units covered by the service level agreement will have been admitted from outside of the Cheshire footprint and therefore will not be registered with a local GP, or eligible for such registration. This service provision allows access to ongoing physical health monitoring/ screening. Better Health Manchester are able to offer a service which aligns with the needs of the three wards, and matches the service provided by the previous provider. They currently work with other NHS Mental Health Trusts and have extensive experience of working with these patient cohorts. The board of directors is multi-disciplinary and has a clear structure of accountability. The team includes a wide range of healthcare professionals to include: Care coordinators, GPs, Healthcare assistants, mental health practitioners, nurse practitioners, pharmacists, physiotherapists and practice nurses.
